Our History
Following a very thorough, thoughtful multi-year process the Great War Memorial Hospital Foundation and the Smiths Falls Community Hospital Foundation, amalgamated to form the Perth and Smiths Falls District Hospital Foundation.
The new name reflects the fact that the new Foundation will be responsible for raising funds for the two sites of the Perth and Smiths Falls District Hospital – one Hospital, one Foundation, two sites. This amalgamation will allow the best elements of both Foundations to make an even bigger impact on the lives of those who need PSFDH – on every step of their journey.
With offices at both sites, we look forward to continuing to work with you funding the best possible care for patients seeking the hospital’s help, by supporting the hospital, its equipment and the amazing people that work there.
While the name has changed, the ways that you can give have not!

Great War Memorial Hospital Foundation
The Great War Memorial Hospital Foundation was a charitable organization established in 1982, to financially support the purchasing of medical equipment and the expansion of services and facilities at the GWM Site of the Perth & Smiths Falls District Hospital. Through active fundraising and community outreach the Foundation served as a conduit of community support, linking donors and the public with the Hospital.
Smiths Falls Community Hospital Foundation
The Smiths Falls Community Hospital Foundation was a registered charity established in 1981 to help meet the growing financial needs of the Smiths Falls site of the Perth and Smiths Falls District Hospital. The Foundation organized fundraisers and worked with community donors to raise the funds needed to purchase medical equipment and modernize inpatient and outpatient facilities that are not funded by the Ministry of Health and Long-Term Care.
