What is the Foundation’s legal name?
Our legal name, and the proper beneficiary for you gift is the Perth and Smiths Falls District Hospital Foundation.
What is the Foundation’s charitable registration number?
Our charitable registration number is 1192 36016 RR0001.
Why do you need to fundraise? Doesn’t the government pay for health care?
We need to fundraise to fill the gaps where provincial funding ends.
How do I become a Foundation partner?
Will I receive a tax receipt for my donation?
While every single dollar is a generous and meaningful gift, tax receipts are provided for donations greater than $20. Your receipt will be mailed or emailed.
Can I designate my gift to a specific program or a department?
Yes, gifts can be directed to a specific piece of equipment, department, or site. We are one hospital and welcome your partnership in any form you choose.
How does the Monthly Giving Program work?
When will I receive the tax receipt for my monthly gifts?
A tax receipt will be issued at the end of each giving cycle (1 calendar year) for all cumulative gifts given from January to December.
Can I run an event on behalf of the Foundation?
When I give in someone’s memory will their family be notified?
Yes. If requested, the family will be notified on your behalf. Please note that the amount of your donation will be kept confidential unless you specify otherwise.
