In Person
We have an office at both sites of the Perth and Smiths Falls District Hospital. Come visit us and make your donation with a credit card, cash, or cheque.
Download, print and fill out our donation form and mail your donation to one of the PSFDH Foundation offices (Perth or Smiths Falls).
